Instructions
To complete the online Event Outcome Form below, firstly choose the type of funding you have received (Sporting Development or Community Chest).
You must leave a contact name and number of someone whom we can contact to confirm and clarify the information submitted. (NB: Contact person must be 18 years of age or older).
Please provide details of the event or project, date and venue, activities or project undertaken, number of people involved, milestones achieved etc.
Please attach a high-resolution digital photo (copyright free) that captures your performance/event/project. (JPEG format with a file size from 1 - 2MB is preferred).
NB: In submitting this outcome form information and photos, you approve of Caboolture Sports Club using this information for its promotional and publicity purposes including website, social media pages, members magazine, reports to stakeholders etc. If you have any privacy concerns regarding the supply and use of this information (especially if you are not the parent or guardian of a young person referred to in within the outcome information or photo/s) then do not submit the outcome form but instead contact Caboolture Sports Club directly to discuss.
